UPON ARRIVAL AT JOB SPUT ALONG WITH MSHA INPSECTOR EE A SMOLDERING FIRE WAS FOUND IN A STOCK PILE OF COAL AT JOB SITE UPON FURTHER INVESTIGATION IT WAS FOUND THAT THERE WAS A HOT FIRE BURNING INSIDE THE STOCK PILE OF COAL.

EE WAS CHANGING THE CUTTING EDGE ON A DOZER. HE WAS HOLDING A WRENCH SO THAT THE NUTS WOULDNOT MOVE THE WRENCH PINCED HIS LEFT MIDDLE FINGER BEHIND THE BLADE AND CUT THE TIP OF HIS FINGER OFF APPROX. 1/2 INCH.
EE WAS DRIVING A ROCK TRUCK OUT OF THE PIT DUE TO SLIPPERY CONDITIONS THE TRUCK SLID INTO THE BERM AND ROLLED ONTO ITS SIDE.
EMPLOYEE WAS WALKING THE PERIMETER OF JOB SITE TO INSPECT THE SILT FENCE. HE STEPPED ON A LOOSE ROCK, SLIPPED AND FELL DOWN ON HIS RIGHT ELBOW.
