Vulcan Construction Materials, LLC · Caught in, under or between a moving and a stationary object

While using a come-a-long to raise and align a loader bucket cutting edge, the hoist hook slid, pinching the employee's gloved finger. Employee was diagnosed with an open phalange fracture, received sutures, and released with restrictions.

EE was attempting to unplug chute with a high pressure water hose. As soon as the EE opened the valve, the pressure caused them to lose their footing, falling backwards against the hand rail, and striking right ribcage on the top rail. EE sought medical attention on 8/30.

Personal health. After inspecting the plant with others for approximately 45 minutes, at the truck the employee became disoriented and passed out for a second or two. Temperature was approx. 84 degrees F. EE was taken to the office where EE quickly recovered. EE stated had a history of fainting. As such, this incident is under investigation to determine work relatedness.

EE was setting up a hose to flush out material under the belly pan (located at eye level). When EE turned on the valve, the pressure caused the hose to move and strike EE in the right eye causing it to bleed.
