EE was attempting to unplug chute with a high pressure water hose. As soon as the EE opened the valve, the pressure caused them to lose their footing, falling backwards against the hand rail, and striking right ribcage on the top rail. EE sought medical attention on 8/30.

EE was stabilizing the crusher door when the right middle finger was struck by a shackle that had slid over and caused an avulsion to the tip of EE's right middle finger.

Personal health. After inspecting the plant with others for approximately 45 minutes, at the truck the employee became disoriented and passed out for a second or two. Temperature was approx. 84 degrees F. EE was taken to the office where EE quickly recovered. EE stated had a history of fainting. As such, this incident is under investigation to determine work relatedness.

EE was setting up a hose to flush out material under the belly pan (located at eye level). When EE turned on the valve, the pressure caused the hose to move and strike EE in the right eye causing it to bleed.

Employee received a hand laceration while trying to remove seal from fuel truck thermostat. He sought medical attention, received sutures and was released to restricted duty. Employee was later terminated due to a history of excessive safety infractions.

EE was moving wooden blocks for support and a block fell smashing left thumb

Employee was operating a piece of equipment (dozer) when he came to uneven ground which resulted in the jarring of his back.

Employee was anchoring a piece of equipment with a "come- along" when the lifting mechanism broke causing the item to strike him in the left thigh resulting in a contusion.

Employee was tightening bolts on the primary plant, when he lost his footing due to muddy conditions. This resulted in him falling onto a conveyor which caused pain to his right knee.

Employee was using a small sledge hammer on wedges when he missed, stricking his right index finger, resulting in a contusion and a subungual hematoma.

Employee was shoveling when he felt pain in his lower back.

Employee was walking under the diester screen, when a rock fell off the belt and landed on his left foot, resulting in a fracture.

Employee was shoveling at the diester screen when he felt pain in his lower back. Employee sought medical on 6/8/09 and Dr. confirmed that employee had sustained a lumbar strain.

Employee was preparing to go upstairs when he slipped on gravel causing injury to right wrist and right knee.

EE was cleaning under a screen when 3 rocks fell onto his right shoulder blade.

EE was using air ratchet tool, attempting to remove bolt from VSI table on crusher when he hit his hand against table.
