Employee was tramming scoop and attempted to angle over to the left to move a bucket insert when the deck of the scoop hit a timber laying in a pile and the timber came into the deck and hit his right hand resulting in a cut to the index finger and a serious cut to the little finger (pinky).

Employee was helping install a hinge pin in the ATRS on a roof Bolter when the blocking slipped and caught his right hand, resulting in a laceration and possible broken bone(s) in the hand. Will have surgery to repair the damage.

a roof fall was found in the #7 entry of No. 1 Southwest Mains 1 break outby spad #607. This is not a regularly traveled area. The fall was approximately 20' long, 20' wide and 5' thick. The area was timbered off.

Employee was unrolling a 3" water line on the surface area of the mine when he slipped and fell onto his left side and landed on his knee, causing it to twist. The employee, injured on 3-3-14, continued to work at his regular job and did not report the incident until 3-6-14. He was sent to Dr. on 3-7-14 and the Dr. placed employee on restricted duty until further evaluation.

Employee was changing the starter steel to a finisher and pusher steel. As he was lining the pusher and finisher steels up he accidentally hit the lift lever and caught his left pointer finger between the two steels. He received medical attention and has been put on restricted duty. Days restricted will be reported when he returns to work.

The number one intake portal was blocked with rock when a portion of the highwall over the drift fell in. No injuries were received. All personnel were pulled from the mine.

Employee got his right foot ran over with the bolter tire. The operator inched the bolter up to get the t-bar straight. Helper thought it was set and he reached for a bolt and got his right foot in front of the tire as the bolter was inched forward.

Employee was riding a 3 wheeled buggy. He was making a pre-shift check and the buggy ran into a small hole in the roadway causing a jolt that caused bruising to his hip and lower back as he landed on his mine rescuer that was on his belt.

As employee was loading roof bolts plates onto a bolter he hit his right arm near his elbow on a bundle of plates lying on the machine and received a laceration that took 7 sutures to close the cut.

Employee was unhooking a set of jumper cables for a scoop battery charger and as he pulled on the cables the cat head came uncoupled and hit him over the right eye causing a small laceration that took two sutures to close. Employee missed no work and is working his regular job.

Employee was stacking a brattice up and as he was installing a door a small part of the brattice fell and hit him. Employee is on restricted duty and the days of restriction will be reported when he returns to full duty work.

Employee was operating the bolt machine installing bolts when his glove got caught in the chuck and he accidentally hit the rotation lever and rolled his fingers around the steel. He had a dislocation middle finger and ring finger and laceration on the left hand. He is on restricted duty and days will be reported when he returns to work at full duty.

On January 3,2012,employee reported a strained abdomen while helping pull rock off the miner. Did not go to Dr. and Company was not aware of any diagnosis. July 7, 2014, employee informed the Company he was having hernia surgery on July 11,2014. After review, it was found that the employee had been diagnosed by his Dr. in March 2012, but Company was not made aware of that.

As employee was cleaning out the deck on the miner a small piece of rock fell and hit him on top of the right shoulder causing a bruise. He stated that he checked the top before starting work on the deck. He was on restricted duty for 5 days.

Employee was sitting in the mantrip that had a buggy cable coming from the power center hanging over head. The cable anchor was taken down to move the cable up. When the buggy was started it jerked the cable down and hit employee in the cap and head straining his neck.He was put on 5 days of light duty.
