EMPLOYEE WAS USING A PROPANE TORCH TO ASSIST WHILE POUNDING ON HOPPER TO LOOSEN FROZEN CHUNKS OF SHALE HUNG UP IN CHUTE. THERE WAS NO WITNESS AND EMPLOYEE DOES NOT KNOW WHAT HAPPENED, BUT THE TORCH APPARENTLY FELL TO THE FLOOR CAUSING A FIRE WHICH BURNED HIS HANDS, ARMS FACE & HEAD
