Employee was grinding on the cone liner using a 4 1/2 in. angle grinder. EE was only using one hand on the grinder as the other hand was holding the electrical cord to keep it out of the way. The grinder kicked back and the wheel make contact with the employee's left pointer finger. EE was taken to the clinic and received 5 stitches and a tetanus shot.

On Monday April 2, 2018, the EE was walking next to a pump house when they tripped and fell to the ground. It was unclear the extent of the injury as the injury was not reported until April 11th, 2018. The EE had swelling and an infection in their knee. The EE received medical treatment to reduce the swelling and infection through antibiotic medication.

On 2/4/15 at 9:00 a.m. two employees were loading a grizzly feeder and jaw dies onto a flatbed trailer. As the die was lowered and tension was let off, the die swung and struck the ground employee in the leg.

The employee was cutting a 60" under screen belt when the knife slipped out of the belt and cut his leg. The laceration resulted in 5 stitches.

Employee operating a water truck was traveling down the mountain haul road when he accidentally shifted into a higher gear causing the equipment to speed up. Employee tried to shift down to lower gear but was unable to. Applied brakes and ran equipment into a berm causing the truck to flip over. Employee was trapped inside for a short period before being rescued.
Adjusting the speed on Eljay screen. Rolling Eljay back to remove belt, when hand was pulled into pulley, crushing his right hand.

There was screen material in the back of a pickup truck that was being used to build a guard. It was snowing and blowing heavily and the employee got up to go to the pickup. He was walking with his head turned to keep the snow out of his face and ran into the screen material in the back causing a laceration to left side of nose requiring 7 to 8 stitches to close.
