LAMINATED ROOF HAD A COAL SEAM ABOVE ANCHOR POINT OF RESIN BOLTS, LOCATED ABOUT 25 CROSS CUTS OUTBY FACE AREA.
SLIPPED ON TRACK WHILE UNLOADING 6" BRATTICE BLOCKS AND FELL. HE STRUCK HIS LOWER BACK LEFT HIP ON THE TRACK.

THE INDIVIDUAL WAS HELPING TO REPLACE A HYDRAULIC HOSE ON HIS ROOF BOLTER MACHINE. A PIECE OF SLATE TYPE ROCK FELL FROM OVER HEAD CAUSING HIM TO DODGE HIS HEAD SIDEWAYS AND HE STRUCK THE CORN ER OF A SIDE LIGHT ONT HE MACHINE WITH HIS RIGHT CHEECK OF HIS FACE. RESUTLING IN A CUT APPROX 1/2 INCH LONG.
