EMPLOYEE WAS INSTALLING ROOF BOLTS IN #3 ENTRY WHEN A PIECE OF DRAW ROCK MEASURING 3 FT BY 3 FT. BY 2 INCHES FELL CAUSING THE EMPLOYEE TO BE PINNED IN SITTING POSITION BESIDE THE ROOF BOLTER. THIS CUASED BRUISING TO THE LEFT SHOULDER AND BACK. THE ROCK FELL BETWEEN THE BOLTS.

HE WAS INSTALLING ROOF BOLTS IN #6 RIGHT BREAK. HE HAD PUT UP THE FIRST RIB BOLT AND WAS PREPARING TO PUT UP THE SECOND BOLT WHEN A PIECE OF DRAW ROCK BROKE OUT BETWEEN THE ROW OF BOLTS HE WAS INSTALLING AND THE ROW OUTBY. THE ROCK FELL ON THE TRAY OF THE BOLTER AND SLID OFF ON THE LEFT FOOT OF THE ROOF BOLTER OPERATOR.

S/C OPERATOR WAS LOADING UNDER MINER BOOM. MINER BACKED UP, PUSHED S/C SIDE WAYS. OPERATOR HAD ARM OUTSIDE OPERATOR'S COMPARTMENT. ARM WAS CAUGHT BETWEEN S/C AND COAL RIB.
