A ROOF FALL WAS FOUND APP 200 FT OUT BY THE SECTION ON THE INTAKE AIR COURSE. THE FALL IS IN A INTERSECTION OF THE #7 ENTRY. THE FALL IS APP 18' BY 70' BY 6' THICK.
EE WAS OPERATING A ROOF BOLTER WHEN A PIECE OF ROCK MEASURING 1"X15"X27" FELL FROM THE ROOF A HIT HIM ON HIS BACK. THIS LEFT A PUNCTURE WOUND ON HIS BACK THAT REQUIRED 2 STITCHES.

EE WAS CHANGING A BELT WIPE ON THE SURFACE BELT. IN THE PROCESS HIS FOOT SLIPPED WHICH CAUSED HIM TO FALL AGAINST THE SIDE OF THE METAL BEAM THAT SUPPORTS THE BELT. THE BELT CAUGHT HIS CHEST F RACTURING ONE OF HIS RIBS.
