SECITON BOX LOCATED IN 2 RIGHT HEADING OF #3 MAIN CAUGHT ON FIRE. VENTILTION WAS ESTABLISHED TO ROUTE SMOKE AWAY FROM BOX. MEN WERE EVACUATED FROM MINE AND FIRE FIGHTING CREW EXTIGNUISHED FI RE. BOX WAS TAKEN OUTSIDE. LASTED APPROX 1 HOUR.

EMPLOYEE WAS IN SQUATTING POSITION WHEN HE SLIPPED ON ICE. TO BREAK HIS FALL HE EXTENDED HIS HAND, HURTING HIS WRIST AREA, THUMB AND 2 FINGERS. ACCIDENT OCCURRED OUTSIDE WHILE EMPLOYEE WAS ATT EMPTING TO MAKE MECHANICAL REPAIRS TO BOLT MACHINE.
