EE was exiting the office and reaching to close the front door when another employee saw the door ajar and went to close the door. When the other employee closed the door EE's left middle finger and left ring finger pinched in the door and caused fracture of distal phalanx and finger lacerations with complication.

While using a utility knife to cut conveyor belt on the Primary Feed Conveyor the employee was cutting towards themselves, the knife slipped cutting through their glove and cutting the left thumb.
