EE OBSERVED LARGER THAN NORMAL ROCK ENTER FEED HOPPER, WHEN ALMOST EMPTY INSTEAD OF TURNING FEEDER OFF HE RUSHED DOWN THE STAIRS TO THE BOTTOM FEED BELT TO CATCH AND REMOVE LARGE ROCK AS IT CA ME OUT AND WENT ON TO MAIN FEED BELT. GOING HALFWAY DOWN THE STAIRS THE ACCESS TO THE MAIN FEED BELT IS THRU A CROSS ACCESS, WHILE TURNING IN A HURRIED DOWNWARD DESCENT, FRACTURED ANKLE.

EMPLOYEE WAS WALKING BESIDE A STATIONARY PUMP MOTOR SUSPENDED BY A CHAIN BEING CARRIED TO BE SET IN PLACE WITH A FRONT END LOADER. A DOWN HILL GRADE WAS ENCOUNTERED. BRAKE FAILURE OCCURRED CAU SING SUSPENDED MOTOR TO SWING ON THE CHAIN STRIKING EMPLOYEE ON LEFT UPPER ARM.
