It was raining. Worker was filling DEF and went to step off of excavator and slipped. EE grabbed a handrail with right hand as they fell and "heard something pop".

Employee was walking around Sandvic QA340 and stepped in a small pothole and fell on the ground. This resulted in injuring a knee, specifically, tearing the ACL.

The operator was standing on the track to access the tool box during the pre-shift. Track was clean and dry. EE stepped to side to open the toolbox and feet slipped off of the side of the track. EE was unable to retain hold on the handrail and fell to the ground. EE was bruised.
