ON 7-9-85 EMP R9PORTED FOR WORK AT 7:00 AM ON HIS REGULARLY SCHEDULED SHIFT. HIS DUTIES WERE TO FEED THE PLANT AND BUILD BERMS. AT APPROX 10:00 AM WHILE DUMPING DIRT TO BUILD BERMS AROUND THE HOLE THAT WAS M5DE FROM EXTRACTING MATERIAL, HE WAS UNABLE TO STOP THE LOADER AND CONSQUENTLY WENT OVER A 20-FOOT EMBANKMENT. HE TRIED TO PUT THE TRANSMISSION IN REV ERSE BUT WAS UNABLE TO.
