At time of filing it is believed a front-end loader struck the back of the tractor trailer. The position of the victim at the time of the accident is still under investigation. The victim was found under the rear driver's side tires of the tractor.

Employee was working on a Raymond Roller Mill when he entered into the roller area of the mill. The mill was energized and the rollers crushed him.

EE WAS FOUND BY A FELLOW EE ON THE GROUND FACE DOWN WITH HIS HEAD TOWARD FORKLIFT TRUCK. THE TRUCK HAD FLIPPED ON ITS SIDE AND THE BAR FROM THE FALLEN OBJECT PROTECTION FELL ON HIS BACK. FROM VISUAL OBSERVATION DURING MSHA INSPECTION IT IS EVIDENT THAT THE FORKLIFTS RT FRONT TIRE HIT A PIPE PROTRUDING FROM THE GROUND FLIPPED OVER ON ITS SIDE.
