WHEN CLOSING A GATE ON CAR ,BAR CAME BACK AND HIT RIGHT SHOULDER.
STEPPED ON ROCK WHILE STEPPING OFF RAIL CAR AND SPRAINED ANKLE.
HE WAS LOADING A TRUCK FROM A LOAD OUT BIN. HE PULLED THE HANDLE ON THE VALVE TO CLOSE IT AND TWISTED HIS BACK IN THE PROCESS. THERE WAS NO PROBLEM WITH THE VALVE.

WHILE OBSERVING/HELPING THE PLANT MANAGER WORK ON AN ELECTRICAL PROBLEM ON THE AIR COMPRESSOR, THE ELECTRICAL CONTACTS WERE PUSHED TOGETHER CREATING A BRIGHT FLASH WHICH BURNT THE EYES SIMILIA R TO WELDERS FLASH.
