Employee had a periodic screening chest x-ray read by two B-readers using the ILO procedures. The x-ray was characterized as consistent with pneumoconiosis with readings of 1/0 and 1/1. Damage or impairment cannot be determined solely by reading chest x-ray films. This report is being submitted to satisfy MSHA program policy letter P92-III-2.

Employee was using a sledge hammer to break a nut loose on a crusher. He complained of back soreness the next day. He did not note having any pain until he was at home that night.

While adjusting belts on a jaw crusher feeder motor employee slipped and fell backwards into the crush frame directly behind him bruising his back and shoulder.
While using a wire brush, dirt flew underneath safety glasses and went underneath EE's eyelid.
EE pulled muscles in his neck and shoulder area while climbing into an excavator.
While replacing parts in the VSI crusher the jack holding the access lid up failed trapping injury's arm leaving it bruised
While attempting to lift access lid on a VSI crusher, employee strained muscles in his shoulder.
While using a grinder repairing a roller clamped in a vice on a workbench, the grinder slipped cutting employee's finger.

While changing a jaw crusher die, the die struck EE on the left knee, while moving the die into place for installation.
Slipped and fell injuring right shoulder.

Employee was using a hammer and a chisle to remove a bearing for a snubber pulley. The hammer broke and hit his left index finger, cutting it and his finger nail. He went to the emergency room and had finger nail removed and 9 stitches. He returned to work at 2 PM.
Employee was grinding and got the grinder against a piece of pipe and the grinder kicked back into his left leg. The grinder cut his leg about 2-3 inches long and 1/4 inch deep and 1/4 inch wide. He had 7-8 stitches on the cut.

WORKING IN SMALL BOAT- FELL WHILE PULLING TO BANK. SMALL SCRAPE, BUT SAID HE WAS ALRIGHT - THOUGHT ONLY BRUISED, BUT HE SAID IT GOT WORSE AFTER A COUPLE OF WEEKS.
EE WAS STANDING ON TOP OF GUARD ON CRUSHER USING A SHOVEL TO UNCLOG A FEED CHUTE. MATERIAL IN CHUTE GAVE WAY, EE'S FOOT SLIPPED, AND EE FELL HITTING ANGLE IRON AGAINST RIB CAGE.
