Using jib crane to reposition pump clean-out/door assembly and part spun towards employee striking left leg and breaking it

Using a circular grinder to cut off the top of a metal handrail. EE had one hand on the rail and was operating the grinder with the other hand. The grinder requires two handed operation. (EE was wearing gloves for protection.)

Working on disconnect knives of a motor starter bucket in a motor control center. Power was on. Employee inadvertently placed screwdriver on energized knife and door saftey catch of motor control bucket. Injury was arc flash burn to right and left hand and face. The said bucket was on the bottom and employee was laying on left side during the repair.

REMOVING BOLTS FROM THE PIPELINE, WRENCH SLIPPED AND HIT IN THE MOUTH. CHIPPED TWO UPPER FRONT TEETH.

CHANGING SUCTION SIDE DOOR ON 10IN PUMP MASHED FINGER

MOVING THE DREDGE WITH LEAD LINES HAD HT EHANDLE ON THE RIGHT LEAD LINE STANDING STRAIGHT UP INSTEAD OF LAYING DOWN. CABLE GOT CAUGHT FLIPPED HANDLE DOWN AND CHOSE HIS RIGHT HAND.

WET LADDER & WET SHOES HAD STITCHES TO RIGHT HAND.

THE EMPLOYEE COMPLAINED OF STRAINED BACK MUSCLES ON 7/10, DID NOT NOTIFY SUPERVISOR UNTIL 7/12, DID NOT SEEK MEDICAL ATTENTION UNTIL 7/17. EMPLOYEE WAS OFF FROM 7/17 TIL 7/21, RETURNED ON 7/21 /93 WITH NO RESTRICTIONS. STRAINED MUSCLES IS APPARENTLY BECAUSE OF MOVEING DREDGE LINES.

EMPLOYEE WAS PULLING CABLE FROM DRUM INTO BOAT TO ANCHOR ON BANK EMPLOYEE STRAINED MUSCLE ON RT SIDE & SHOULDER

SPRAINED ANKLE STEPPED DOWN FORM SLAB GROUND WHERE FAULT CONTROL BOX IS ON

EMPLOYEE WAS REMVOING THE DOOR FROM THE SAND AND GRAVEL PLANT WHEN THE DOOR FELL, PINNING THE EMPLOYEE BETWEEN THE DOOR AND THE PIPELINE.

ROLLING 10" LINER FOR PUMP SHELL ONTO DREDGE BY HIMSELF. LOST CONTROL AND FELL AGAINST LEFT ANKLE. CAUSED SPRAIN AND BRUISE.
