Employee parked truck part way on ice. Got out and saw the ice but still walked behind the truck on ice and fell getting a wrist sprain. There were areas without ice.

Working on disconnect knives of a motor starter bucket in a motor control center. Power was on. Employee inadvertently placed screwdriver on energized knife and door saftey catch of motor control bucket. Injury was arc flash burn to right and left hand and face. The said bucket was on the bottom and employee was laying on left side during the repair.
