Employee drilling a 1/2" hole. Drill bit snagged a thin wire and wrapped it around. when this happens the loose end of wire away from drill bit wrapped around employee finger causing a laceration

Employee was re-tightened a hose connection on the ground next to a 300 cone. Gathering belt was feeding the 300 cone crusher with 5-7" rock. A rock bounded out of cone and landed on employee left hand while they were re-tightening the water hose.

Employee was kneeling down near a switch gear box. Another worker was getting ready to turn on switch, when EE did it made a noise and startled employee and caused employee to jerk left left arm, pain followed immediately after this in the shoulder.

Employee operating a water truck, drove to pond to fill up. Drive way at pond has some water that was from overfilling water truck. Employee got out of truck, when EE stepped onto the puddle EE didn't realize that there was a pot hole and lost EE's balance and fell down hurting EE's right ankle.

Two man operation, Changing 5/8 screen on # 2 screen feed, employees were placing screen in place on top of screen deck, while pulling screen in place the screen slipped and pinched right pinky finger, between screen and head pulley guard.
