EMPLOYEE WAS USING A PRY BAR FROM AN OVERHEAD POSITION TO INSTALL A VERTICAL TRAVEL SHAFT IN A MANITOWOC MODEL 4500 DRAGLINE THE PRY BAR SLIPPED OUT OF POSITION AND EMPLOYEE JERKED HIS HEAD T O AVOID HITTING HIS HEAD AGAINST THE MACHINE AND EXPERIENCED STIFFNESS AND SORENESS IN NECK MUSCLES.

WAS REPAIRING DRAGLINE AND STEPPED ON DRUM BANDS. THE BANDS WERE OILY; SLIPPED AND FELL BACKWARDS ON HIS HIPS, SINCE THEN PAIN ACROSS HIPS, WHICH EXTENDS UP TO RIGHT SHOULDER BLADE. HAS NUMBNE ESS IN RIGHT SHOULDER AND RIGHT ARM. DULL HEADACHES, SINCE THE FALL.
