Employee said they were lifting a box of 8 gauge shells weighing 50 lbs. and felt pain in their lower back.

Employee was walking around screen conveyor tail pulley when EE tripped over the guard and hit EE's knee.

Employee was using a utility knife to cut a piece of 6" rubber skirt board.

Employee was cutting an old chute down in pieces and putting them on the ground when a sharp edge cut EE's finger.

Employee was carrying an acetylene tank when they felt a pulled muscle.

Employee removed the guard on the 30" stockpile conveyor snub roller. While it was running, EE put a crowbar in it to clean.

Employee was using a crowbar to open hatch in the tunnel when the bar slipped and employee hit elbow.

Tripped on a harness safety lanyard

Employee was loosening bolts on the mill when they lost their grip and hit their arm on a piece of metal.

Employee was hitting a coal hopper with a 3lbs lump hammer and hit/jammed their finger.

Employee was hitting slug wrench with hammer when the nut came loose the wrench hit EE's finger.

Employee was cleaning up material at the finish plant. EE walked up to the shop and told the supervisor to send the maintenance down to help when the super said the EE said hurt knee. *The EE was denied a vacation day for Friday.

Employee was picking up a piece of 2x2x1/4" angle iron when EE dropped it on their finger.

Employee was walking when they tripped and fell, cutting their right palm.

Employee was walking when the wind blew dust in EE's right eye.

When turning down head nut with pinch bar in shop, strained left shoulder.

Employee was walking under #1 kiln by tunnel entrance when employee *said* employee slipped on ice. *NO ICE WAS FOUND!

Employee was wheeling brick in kiln when employee said employee felt a pain.

Employee was replacing guard on Portable Plant head pulley conveyor

On 11/26/18 EE was asked to help maintenance employee then said couldn't do it because EE hurt back on 11/21/18. This was at start-up.

Walking to EE's pickup truck, lunch time, twisted left ankle. Note, in employee parking lot.

Check kiln operations stepped on empty shell casing while adjusting air door.

Employee opened breach door on rear of #1 kiln. Walking down stairs, lost footing, missed step.

While climbing #1 bucket elevator to grease on top, dust or stone got in left eye.

Walking from bathroom by the back door of #2 kiln on the west side stepped on rock or rut

Employee lost footing and fell

EE said he was moving the kiln gun away from the kiln after shooting a ring out and slipped off diamond plate and hurt his pinky.

Putting a conveyor belt splice in the 3/4 conveyor @ the finish plant.

A breaking switch located in the portable plant control room shorted out causing a flash where the breaker was turned on causing minor burn to employee's hand.

EE was climbing on loader to add engine oil to equipment when he said he pulled something in his hip.

EE was welding on #2 elevator when he started to weld got a shock.

Shoveling in #2 tunnel said he twisted knee

Working on rebuilding hydraulic cylinder. When tightening nut, wrench slipped off nut pinching right pinky finger on floor.

Employee was working on top of #2 Elev. when co-worker opened the breach door letting dust out in area where employee was working.

Climbing out the kiln room window to gain entrance to catwalk to work on walls.

EE was working on adjusting auto brake control when the wrench slipped off the nut hitting his elbow. No lost time from work.

EE working on bolting down the symonds crusher motor at the finish plant holding the bolt on the bottom and turning the top with a impact wrench the wrench turned and jammed his left thumb into the frame.

Standin gby #2 Ballmill & pit area, when wind blew debris in left eye.

EE was putting out a fire on the coal conveyor.

Employee was working in #1 kiln bricking. When he climbed down from the brick rig he twisted his right knee on something, he's not sure what it was.

Moving an oil drain pan from under a wheel loader, he felt his shoulder pop. Note: we were not made aware of this until 9/25/08.

Employee was cutting a 2x4 with a skill saw when the guard stuck up.

attempting to remove a 6"x7' pc of 1/4" steel from the apron feeder @ the raw plant. winch had stoved emergency. When the pike let go it hit ee in the left cheek.

Cleaning rollers on conveyor belt, caught hand betwen sledgehammer and metal conveyor support.

EE was driving in front gate on his way to work with personal vehicle with drivers window down when a dust partical went in his eye.

Employee was cleaning the threads on a 1/4" pipe nipple 6" long when the bench mounted grinder wire wheel caught his thumb. He said that if he was wearing his gloves he would be ok. (Note clearing on rest was good at this time.)

Slipped on ice, hurt left shoulder.

EE was cleaning mud from the 30" stockpile chute at the raw plant when he said he pulled a muscle in his lower back.

WORKING ON JAW CRUSHER REPLACING TOGGLE PLATE, STANDING PLATE ON END WHEN EMPLOYEE'S RT. POINTER FINGER BECAME PINCHED BETWEEN PLATE & "I" BEAM SUPPORT.

CLIMBING IN SCREEN DECK AT THE PORTABLE PLANT & HE HIT HIS RT. KNEE ON A PIECE OF STEEL.

WALKING DOWN HILL & SLIPPED ON ICE.

WORKING ON TOP OF KILN GRINDING SURFACE TO BE WELDED, HITTING LEFT LEG WITH GRINDER WHEEL, REQUIRED 7 STITCHES.

ATTEMPTING TO UNPLUG THE BALL MILL WHEN COAL DUST IGNITED CAUSING A FLASH FIRE.

WHILE CLEANING A CHUTE, BLOCKED WITH STONE A ROCK FELL FROM THE CHUTE AND STRUCK HIS SHOULDERCAUSING CONTUSION TO LT. SHOULDER AND SHOULDER STRAIN.

HE WAS LIFTING MATERIAL TO REMOVE IT FROM THE AREA AROUND THE DURON STRUCTURE. HE FELT PAIN IN LOWER BACK AND LEGS.

FINE DIRT UNDERNEATH CRUSHER LODGED ON FRAME. WHILE CRAWLING UNDER THE CRUSHER ON THE CONVEYORBELT, MATERIAL FELL INTO EMPLOYEE'S EAR. AS A RESULT AN INFECTION OCCURRED.

HE WAS WORKING INSIDE A KILN, USING A JACK HAMMER; HE SLIPPED ON LOOSE DEBRIS AND HIT HIS LEFT ELBOW AGAINST KILN REFRACTORY.

EE REPORTED TO HIS WORK STATION (KILN FLOOR) AT THE BEGINNING OF HIS SHIFT (1600-2400). ENROUTE TO HIS WORKSTATION, IN FRONT OF THE KILNS HE BREATHER IN SULPHUR GAS. THROUGH THE QUANTITY WAS S MALL, HE REQUESTED AND WAS GIVEN PERMISSION TO GO TO A MEDICAL DOCTOR.

WHILE MOVING A HEAVY OIL BURNER FROM KILN 1 TO KILN 2 EMPLOYEE TRIPPED AND FELL. THE OIL BURNER IMPACTED ON HIS RIGHT FOREARM, TRAPPING IT AGAINST THE STEEL DECK FLOOR. DOCTOR VERIFIED HE HAD BROKEN BONE IN RIGHT FOREARM.

WHILE WHEELING A WHEEL BARROW TO KILN HE CAUGHT THE WHEEL BARROW ON THE RAMP. AS A RESULT OF THE QUICK STOP HE GRAZED HIS SHIN ON THE RIGHT LEG

EE CLAIMS THAT ASHE LIFTED HIS RIGHT LEG UP TO STEP ON A LADDER RUNG IN ORDER T CLIMB TO THE FINISH PLANT SYMONS CRUSHER PLATFORM, HE FELT A A PAIN IN HIS LOWER BACK

EMP WAS STANDING ON A 100' X 100' TARP, PRIOR TO REMOVING IT FROM A 3/4 - #4 MATERIAL STOCK PILE. THE WIND LIFTED THE TARP CAUSING THE EMP TO FALL TO THE GROUND ON HIS RIGHT ELBOW AND JAMMING HIS RIGHT SHOULDER.

EMP WAS USING AN ELECTRIC POWERED HAND DRILL TO DRILL HOLE IN PIECE OF METAL. PIECE OF METAL WASSECURED IN BENCH VISE. DRILL BIT SEIZED IN THE METAL, TRANSFERRING DRILL TORQUE TO DRILL. EMPS HAND WAS IN DRILL HANDLE AND WAS TWISTED.

EE CLIMBED OVER A COAL CROSSOVER CONVEYOR, TURNED TO OPEN A COAL BIN SHED DOOR & TWISTED HIS LEFT ANKLE.

EE COMPLAINED OF A PAIN IN HIS BACK AFTER ASSISTING ANOTHER EE PICK UP A ROLL OF NEOPRENE RUBBER& CARRYING IT TO A WAITING LOADER FOR TRANSPORT.

EE WAS ATTEMPTING TO FALL A TREE WITH A CHAIN SAW. HE LIFTED HIS LEG OVER A DOWNED LIMB AND THE SAW BLADE GRAZED HIS LEFT THIGH. THE RESULT WAS A CUT REQUIRING 26 STITCHES TO CLOSE.

EMPLOYEE MOVING FROM KILN OBSERVATION POINT TO KILN ELECTRICAL CONTROL PANEL TO MAKE ADJUSTMENTS ON KILN BURNING RATE, TRIPPED OVER OLD POST BASE SUPPORT THAT WAS WELDED TO FLOOR, FELL ON RIGH T KNEE ON CATWALK, BRUISING IT.

EE WAS WALKING ACROSS YARD, WHEN HE COLLAPSED.

EMPLOYEE WAS CUTTING A PLASTIC PAIL, WITH AUTILITY KNIFE, WHEN THE KNIFE SLIPPED, CUTTING HIS LEFT KNEE.

EE WAS ATTEMPTING ADJUST THE DRIVE BELTS ON A 5- 1/2"{ STANDARD SYMONS CRUSHER. HE STEPPED ON TO THE TOP OF THE CRUSHER LUBE OIL TANK TO ALIGN A DRIVE BELT. HE SLIPPED AND FELL OFF THE TANK AP PROX. 7' TO THE GROUND, LANDING ON HIS FEET AND INJURED HIS LEFT ACHILLES TENDON.

WHILE SHOVELING SPILLAGE OUT FROM UNDER THE TAIL PULLEY OF A RETURN FEED CONVEYOR AT A CRUSHING AND SCREENING PLANT, THE EMPLOYEE'S HARD HAT FELL OFF. HE BENT DOWN TO RETRIEVE IT AND AS HE STR AIGHTENED UP, HE STRUCK HIS HEAD AGAINST THE CONVEYOR FRAME. THE RESULT WAS A SCALP LACERATION REQUIRING TWO STITCHES.

ON THE MORNING OF 11/01/93 EMPL.OF RAM TRANSIT LINES,INC.WAS PROCEEDING TO UNLOAD A LOAD OF COAL AT THE NE SOLITE CORP. IN MT.MARION N.Y.EMPL WAS UNFASTENING THE TARP ON HIS TRAILOR WHILE ST ANDING ON THE RUBRAIL HE LOST HIS FOOTING FELL FROM THE TRAILOR LANDING ON HIS BACK INJURING HIS HEAD & ELBOW.IF ANY MOR INFO IS NEEDED FOR THIS REPORT PLEASE CONTACT ME.

WHILE REMOVING PREHEATER TUBE FROM KILN H OOD OPENING THE TUBE MISSED ITS FRONT STAND & THE REAR STAND KICKED BACK CAUSING EMPLOYEES LEFT THUMB TO BE PINCHED BETWEEN THE REAR STAND & PREHEATER TUBE

EMPLOYEE CLIMBED ONTO THE TOP OF INISH PLANT HOPPER TO CLEAR OFF THE SCALPING GRID HE STEPPED ON LOOSE COAL CLINKER & SLIPPED FELL BACKWARDS OFF THE HOPPER & STRUCK HSI HEAD ON A PIECE OF CLIN KER

THE BURNES TO HIS FOOT OCCURRED AS HE WAS TRYING TO OPEN KILN BEACH DRAIN DOOR TO EMPTY THE BREACH. HE CLAIMS IT WOULD NOT OPEN. RATHER THEN FIND A TOOL WHICH WOULD DO THE JOB SAFTEY OR FIND A SUPERVISOR AND NOTIFY HIM OF THE PROBLEM, HE STOOD ON THE HANDRAIL AND KCKED IT OPEN. THIS PRACTICE IS WITHOU QUESTION AN UNSAFE PRACTICE. AS THE DOOR OPENED THE HOT DUST RAN INTO HIS UNT

EMP BENT DOWN TO PICK UP A HEAVY V BELT SHEAVE WHEN HE STRAIGHTENED UP HE CLAIMED TO FEEL PAIN IN LHIS LOWER BACK.

THE EMPLOYEE ATTEMPTED ACCELERATE THE STOPPING OF A PUMP DRIVE BY REACHING BEHIND THEW V-BELT GUARD AND HOLDING THE V-BELTS.THE EMPLOYEES 1 ST AND MIDDLE FINGERS WERE CAUGHT AND LACERATED BETW EEN THE V-BELTS AND SHEAVE BEFORE HE COULD PULL HIS AHND AWAY.

THERE MUST HAVE3EXISTED A STRAY IGINITON SOURCE SOMWHERE IN THE COAL DELIVERY SYSTEM. CAVING THE PULVERIZED COAL IN THE SYSTEM TO IGINITE AND DAMAGE THE COAL DELIVERY AND HOUSING. THE INCID ET IS UNDER INV9STIGATION AT THIS TIME.

REMOVING TIRE &3WHEEL FROM HAUL TRUCK-WRENCH SLIPPED STRICKING EMPLOYEE ON FOREHEAD.

MINE FIRE-EMPLOYEE ALERTED ME TO AN UNUSUAL ODOR IN THE OUTSIDE AIR I INVESTIGATED AND FOUND THAT THERE WAS NO STEAM EMISSION FROM PUMP THAT SUPPLIES WATER TO THE SCRUBBER HAD FAILED AND THAT FAILED AND THAT THE SCRUBBER OVERHEAT ALARM HAD NOT SOUNDED THE ALARM IS INTERLOCKED SUPPOSED TO AUTOMATICALLY SHUT DOWN IN THE EVENT OF A PROBLEMS OF THIS NATURE.
