The miner was exiting loader and slipped and fell to the ground, breaking right hip. Miner was taken by ambulance to the hospital and had surgery that night.

While removing the bolts from the counterweight of a Hitachi ZX850H, the counterweight fell backwards onto employee's lower right leg, severing it above the ankle.
