Employee was assisting with installing a tail pulley on P-3. The tail pulley rotated and pinched the tip of EE's right pinky finger. The would required stitches to close.

An employee had an impactor liner, approximately 70 lbs, setting stationary on the impactor platform while changing worn curtain liners in the machine. The liner became imbalanced and began to fall to the catwalk. The employee moved to reposition the liner but was unsuccessful causing a sprained lumbar in EE's lower right back area.

A mobile equipment mechanic was investigating a failed hydraulic line on the 980G loader. The mechanic was standing on the rear offside step and slipped, causing left forearm to slide between the alternator and the fiberglass radiator guard. The contact caused a laceration to forearm.

Injured was replacing a bushing on a Symons 4.25 crusher. As they descended the fixed caged ladder and stepped to the ground they landed awkwardly and suffered back pain. EE sought medical treatment,was diagnosed as lumbar strain. Given pain medication and muscle relaxers and instructed to rest for 2 days. EE's total time working at this quarry was one day.

Employee was tightening a bolt on a wear plate on a 980G Front End Loader. The hammer employee was using to hold the bolt slipped, causing the bolt to strike right middle finger while being spun by an impact wrench. The contact caused a laceration requiring 7 sutures to close. The employee was not wearing gloves at the time of injury.

Employee was attempting to dislodge a section of liner that was jammed in the cone crusher. When ee struck the Manganese liner with a hammer, it fragmented; and a piece hit right index finger causing a laceration.

While evading police on 3/16, a man entered mine property and fell into a sink hole that entered old underground mine workings. Man was transported to hospital. Mine personnel were made aware of the event on 3/20 and notified MSHA immediately. Deteriorated fence around the hole was replaced.

Personal health issue, not work related. Employee was receiving site specific hazard training at the mine when the employee collapsed and started having seizures. The employee was transported via ambulance to the local hospital. Employee returned to work without restrictions two days later. The employee had a history of seizures.

Employee was climbing an extension ladder to secure the top of the ladder, which was leaning against a conduit rack. The employee was standing on the 9th or 10th rung when the ladder feet slipped, causing the ladder to slide down the conduits. Employee rode the ladder down, causing a sprained left hand and sprained right foot.
