Oregon Resources Corporation · Fall to the walkway or working surface

Contracted security guard was performing nightly walk around premises. He was walking between office & maintenance shop & tripped over the handicapped parking curb in front of the office falling onto the concrete. He sustained several broken bones. The light located on outside of maint. bldg was illuminated but light was blocked slightly by shop truck parked in front of bldg.

Oregon Resources Corporation · Over-exertion in lifting objects

Employee was cleaning up plant spillage into a fork lift mounted self-dumping hopper. Latch on hopper was not operating properly and employee repeatedly lifted on hopper to unlatch it. Employee reported that his right shoulder hurt from the lifting at end of shift. Placed on light duty with no heavy lifting.

Employee was in the locker room pulling on his coveralls before the start of his shift when he felt a pain in his lower back. Employee suffered a lower back strain.

Injury was initially recorded as a non-reportable first aid. Employee came back from scheduled days off complaining that he had pain in his RT elbow. Employee was pulling a 30 foot long x 1" plastic add water pipe from surge tank when he felt sharp pain in his RT elbow. MRI taken on 2/16/12 revealed slightly torn biceps tendon. EE is on modified duty for 6 weeks.

Adjusting conveyor belt with 3/4" ratchet it was tight and pulled hard. Employee made a note on his time-card but did not tell anyone. Time-cards submitted days later. Sup. asked employee about note on T/C but employee indicated all was fine only that his shoulders hurt. Did not work 11/30/11 then came in 12/1/11 saying he saw a Dr. and was off work until he could have an MRI.

Employee was working nearby the fluid bed dryer when something went through ID fan causing a loud noise. Employee suffered trauma to her left ear.

Employee was carrying a steel dolly wheel down a flight of stairs in the Dry Mill from the 4th floor. Employee apparently stepped awkwardly on the last step spraining his left ankle. Originally classified as first aid only, but ankle did not improve. Was seen by off-site doctor on 8/17/11.
