Was removing plastic from underneath front end of F350 dump truck, lying on creeper and prying out on plastic. Got bad pain in right side of neck that radiated down right arm.

Ebensburg Power Company · Fall to the walkway or working surface

On April 4th EE told me he had a Dr appointment on April 9th that he had fell on March 21st and his shoulder hurt. Dr sent him for a MRI on April 16th. MRI showed tore rotator cuff, operation will be on May 2nd. He has worked since March 21st with only time off for Dr appointments.

Employee was walking around belt when he hit his head on platform. He worked the rest of the week and reported to me on 6-8-2012 that he had a stiff neck. He went to a chiropractor after work on 6-8-2012 and call me later and said he had a pinched nerve in his neck.

Employee was removing a screen from the screening plant when he felt a pain in his left elbow. He reported it to me on 6/6/08. He is going to have it checked by family dr the week of 6/9/08.
