Shoveling in the tunnel and tried to break a frozen lump loose. It rolled down and struck his ankle.

Employee reported that he had pain in his right arm. He said he did not recollect when or how he hurt it, but believed it happened while operating one of the quarry trucks.
Hooked up hose from lead pot to propane tank, turned it on and lit with striker. it popped in his face.
EE has a Dr who told him his back/neck problems result from working at quarry. No accident occurred and no accident report was filed with company. EE is still working, he has not been told by WC Dr not to work. He is doing physical therapy.
Installing beating roller using a come along, it slipped and fell on his hand.
Employee slipped and fell on the ice.
