Two employees were driving a shaft thru a gearbox to remove it. The injured employee was holding a pipe while another employee hit the pipe with a hammer to drive the shaft out. The pipe was driven sideways by a bad hammer hit and pinched the employees finger between the pipe and pulley, resulting in a laceration and fracture of the left ring finger.

EE tripped while descending a ladder into the Feed hopper of the primary jaw. EE's foot got caught on the extension ladder rope, causing the fall of approximately 5 feet to the feeder floor. The ladder was secured at the top to prevent movement.

Employee was cutting weeds and small trees by plant 3 cross-over and EE said EE was close to the edge of the hill. EE mis-stepped and fell down the hill landed on a small tree which penetrated EE's inner thigh. Six sutures were required to close the cut.

Employee was hooking rigging to a cat walk for removal, and stepped in a hole made by the disassembly process. In doing so, he contacted the front edge of the hole with his shin. This cut his leg, which required stitches.

EE was grinding on the ware metal of the Volvo loader bucket. He was wearing safety glasses and a shield, but he was grinding a piece over his head and got a metal fragment in his left eye.
EE's were putting the crusher back together after changing liners. EE was checking the space on the main shaft when the inner liner shifted and dropped pinching his fingers.
EE was lifting and stacking 50lb. bags of grit when he felt pain in his abdomen.

Two EE's were changing screens on plant III. Injured EE pinched his fingers between two screens when they were clamping them in place.

EE was lifting new screens from the manlift to the third plant screening tower, when he felt a sharp pain in his lower back. The pain eased after a minute, so he kept working. The next day the pain was worse, so he went ot a chiropractor. A couple of days later the pain was worse, so the chiropractor sent him to an MD.

Employee bent over to pick up a 50 lb. bag of grit from the floor & he strained a muscle in his lower back.
EE was driving his haul truck on the haul road when he hit a bump and turned or twisted wrong in his seat. He finished the day with some pain, but the next day was worse. He went to a chiropractor and was told he had a pinched nerve in his lower back.
