I.NAME WAS CHEC5ING THE FUSES IN A 30 AMP 440 VOLT DISCONNECT BOX. THE BOLT THAT FASTEWNS THE GROUNDING LUG TO THE DISCONNECT BOX CAME OUT & ALLOWED THE GROUND WIRE TO MAKE CONTACT WITH ONE T OF THE 440 5OLT PHASES. THE GROUND WIRE BLEW UP & BURNED THE VICTIMS HANDS.
