ROOF FALL OCCURRED #5 ENTRY 2 BRKS IN BY SPAD 632, FALL MEASURED 17 FT WIDE BY 25 FT LONG BY 7 FT THICK. THIS AREA WAS BOLTED WITH 72" TORQUETENSION BOLTS. FALL DID NOT INTERFERE WITH MINE VENTILATION.
MINE FOREMAN MADE FIREBOSS EXAM, DISCOVERED ROOF FALL #5 ENTRY, 65' INBY SPAD 619. FALL MEASURED 40' X 18' X 8'. FELL ON TOP OF ROOF BOLTER. DID NOT INTERFERE WITH MINE VENTILATION.
