WE CUT INTO AN 2LD ABANDONED MINE AND GOT INTO SOME BAD AIR TWO OF THE MEN FELT DIZZY AND GOT BACK IN FRSH AIR THERE WAS AN INSPECTIOR ON THE JOB AND GOT THE NECESSARY EQUIPMENT AND 4 MORE INS PECTORS TO CHEC5 IT OUT. NO ONE WAS INJURED.

IT WAS ABOUT 70 FT FROM THE FORCE OF #5ENTRY SITWAS ABOUT 60FT LONG 18FT WIDE AND 6OR 8FT THICK IT WAS A SLIP THAT HAD BEEN LOGGY AND HAD WATER RUNNING OUT OF IT. ENTRY.
THE PINNER WAS BOLTING IN #5 HDG OF THE SECOND RT SECTION WHEN A PIECE OF LOOSE DRAW ROCK APPROX 4" THICK 2\ WIDE & 4\ LONG FELL FROM BETWEEN THE LAST ROW OF BOLTS & THE ATRS. WHEN THE HELPER JUMPED BACK AWAY FROM THE ROCK HE FELL ON HIS BACK BRUISING BOTH SHOULDERS. THE ROCK DID NOT STRIKE HIM.
BOLTER WAS PINNING IN #5 HDG ON THE SECOND RIGHT SECTION WHEN A PIECE OF DRAW ROCK APPROX 4" THICK 2\ WIDE & 4\ LONG FELL OVER THE PINNER HEAD BETWEEN THE ATRS SYSTEM & THE LAST ROW OF BOLTS. WHEN THE OPERATOR JUMPED TO GET AWAY FROM THE ROCK HIS LEG WAS FOOLED AGAINST THE PINNER & HIS BODY WEIGHT BROKE HIS RIGHT LEG. THE ROCK DID NOT HIT HIS LEG ANYWHERE.
SHUTTLE CAR OPERATOR WAS TRAMMING CAR TO FACE WHEN CAR HIT A HOLE IN THE BOTTOM AND BOUNCED THE IPERATOR UP. HE HIT THE TOP OF HIS SHOULDER ON THE CONOPY OVER HIM.
ROOF FALL OCCURRED SOMETIME OVER WEEKEND OF 7-21-84 FOUND ON 7-23-84 ROOF FALL AT SS#372 FALL 50\ LONG 20\ WIDE 10\-12\ THICK FALL WILL NOT BE CLEANED UP X-CUT OUTBY BELT TAILPIECE.

IT WAS INA BREAK ON INTAKE ABOUT 4 BREAKS FROM FACE IT FELL ABOUT 4FT THICK 18 WIDE AND 5 FT LONG WATER PRESENT OUT OF THE TOP AND IT CREATED A 36FT FAULTY CONDITION THERE HAD BEEN LONGER BOL BOLTS SPRATED ON BOTH SIDES OF IT AND INSPECTOR ALSO INVESTIGNETED.
IT WAS A SLIP AND IT WAS IN RETURN IN OLD WORKS WE HAVE PUT 5FT BOLTS IN IT WHEN STARTING A NEW SECTION INSPECTORS SAID NOTHING COULD HAVE PREVENTED IT IT WAS APPROX 6FT THICK AND 80FT LONG T HERE WAS NO CRACKS VISABLE IN TEST HOLES
