THE ROOF BOLT HOLE HAD BEEN DRILLED AND THE BOLTER OPERATOR WAS GOING TO COMPLETE THE TASK BY PUTTING THE ROOF BOLT IN THE HOLE. WHEN THE BOLTWAS PLACED IN THE HOLE, THE ROOF BOLTER OPERATOR STATED THAT HE HIT THE WRONG LEVER, WHICH TURNEDON THE ROTATION HEAD. WHEN THE ROOF BOLT STARTEDTO TWIST IT INJURED THE BOLTER OPERRATORS HAND. STATEMENT OF ACCIDENT WAS TAKEN FROM MINE FOREMA

THE S & S 480 SCOOP WHICH VICTIM WAS OPERATING WAS DAMP FROM MOISTURE IN MINES. THE FLOOR OF MINES WHERE VICTIM WAS STANDING WAS ALSO WET AND MUDDY. VICTIM SLIPPED WHILE CLIMBING IN OPERATOR'S DECK AND BANGED HIS RIGHT ELBOW. IMPAIRMENT UNKNOWN AT THIS TIME.

VICTIM NEDER REPORTER ACCIDENT UNTIL 8-20, VICTIM WAS ABSENT FROM WORK 8/17, TO 8/23. VICTIM SAID HE REMOVED KETTLE BOTTOM FROM HAULAGE WAY AND PULLED MUSCLE IN HIS BACK HE CAME BACK TO WORK 8 -23- AND RANCAR TOP APPROX. 1 HR OR 60 MIN SAID HE COULD NOT WORK HIS BACK WAS STILL HURTING.
