EE stopped the mantrip to open a set of airlock doors. The mantrip rolled back and pinched EE's left leg against the door causing a bruise. EE was taken to ER for evaluation and returned to work the next shift. EE worked a partial shift on the 21st and has been on light duty until further evaluation by doctor.

Building wooden cribs in an intersection outby the face. While stacking cribs EE felt sharp pain in lower/mid back. EE went to the ER for x-rays, EE is currently off work awaiting further appointments.

EE was helping replace drag link on shuttle car. While holding the drag link, the other helper was using a pipe wrench to twist. Wrench slipped off and caught the tip of EE's right index finger between the wrench and drag link. EE went to the ER and was released to come to work the next workday, worked until 01/22/2022 and returned to the doctor and a decision was made for surgery.

The #3 left entry was cutting into #2 and the fall happened in the #2 intersection of MMU 001. Approximately 20' x 20' over 4' thick. No personnel injury's associated with this rock fall.

Shuttle car operator missed a turn, had to reverse direction and backed into the mine rib. EE stated EE struck head and hurt back inside canopy. EE did not change seating position before changing direction. EE was brought to the outside and drove self home.

EE was loading supplies on the roof bolter. When EE twisted EE felt pain in EE's lower back. Returned to work on 9/27/2021 and worked until 10/02/2021. EE is awaiting further medical advice.

Miner operator stated they were pulling miner cable and felt soreness in back. Finished the shift with no complaints. Stated after a few days they went to the Emergency room and was told to see their regular doctor. On 10/22/2020 employee was scheduled for MRI for evaluation.

A contract coal truck driver exited truck at the fueling area. The loader operator found contractor on the ground laying beside door. Apparent heart attack from underlying health issues.

While pillaring a rock fall happened in the No.1 entry. The continuous miner was caught in the rock fall. The power knocked around 7:45 pm, attempts to move the continuous miner failed within the one hour limit.

EE and the mechanic helper were carrying a gear for the miner planetary. EE stumbled forward and felt pain in lower back. EE was off work with holidays, EE started EE's regular shift on 12/2 stated was still having back pain. Returned to doctor for further evaluation, EE was removed from work starting 12/3/2019 until results from testing.

EE was using a slate bar to pry the chain guard loose at the #9 Belt Drive. The bar apparently slipped off and struck EE's left shoulder and jaw. EE walked to the working section and reported to the foreman. EE was brought outside and driven to the local hospital for evaluation.

Examiner was checking water pumps and found a roof fall in the #5 entry intake, Break 69. The area had already been cribbed and dangered off. The fall was contained within the cribbed area.

Shuttle car operator said they struck boom of continuous miner with car. The operator reported to foreman some pain in left shoulder. Continued to work, saw a doctor 9/10/2019 to be checked, Doctor determined possible damage in neck shoulder area. Notified on 10/11/2019 operator will be off work awaiting procedure.

One of the two roof bolter operators moved the machine trying to straighten it before the starting the row of bolts. The tram side operator moved the machine striking EE's right knee. EE was taken to the local hospital where no broken bones were present. Possible ligament damage awaiting further specialists review.

EE was waiting on the roof bolter to tram up to the next row. The right rib rolled off and struck EE's right ankle. The rib was approx. 4" thick and 42" high. EE was taken to the local hospital where a possible light fracture or sprain was diagnosed. EE is awaiting an appointment with an orthopedic doctor for further evaluation.

EE was cutting the wear strips out of a continuous miner in the surface maintenance shop. In the process of cutting, the wear strip fell unexpectedly and struck right hand. The impact caused a cut and bruising to right hand. EE left hand showed small bruise.

EE was loading roof bolt resin onto the roof bolter and felt pain in lower back. EE continued to work every shift until 2/19/2019. After reporting to a physician it was determined as a lower back strain. EE has to take time off until therapy is complete.

EE told foreman after arriving on the section ee needed a ride outside back was hurting from a previous injury at another mine, ee stated had been on high powered pain medicine. EE told the foreman and the superintendent no accident happened here. EE left the mine and never returned to work or called back. Only worked 2 full days after hire.

EE was carrying an 80lb bag of Redi-mix concrete, while in motion EE felt lower back and leg pain. EE worked the rest of the shift and worked every shift until 7/11/2018, reported to a doctor and is off awaiting evaluation.

EE was pulling small pieces of loose rock off the top of the continuous miner. A rock EE was moving slipped from EE's left hand and pinched the left index finger on the side of the machine. The impact caused a cut to EE's left index finger requiring stitches.

EE was stepping out of the shuttle car to change direction and twisted left knee.

EE was traveling the return and found a roof fall in the old south mains near spad number 7047. The fall was not in a regular travelway and no air restrictions were found. No accident or injuries were found. The fall was approx 80'x 19'x 4-8'thk.

EE was working to repair the broken conveyor belt on the #5 belt line. As they were pulling on the conveyor belt a chain hook slipped causing the chain to strike a roof jack pipe. The roof jack pipe struck the victim in the face causing facial fracture, nose, jaw, neck.

Miner operator was moving a 2/0 cable out of the roadway in the intersection of #2 entry. As operator was turning/ twisting heard a pop sound in left knee. The pain became intense after that point.

He was cutting wood timbers with a hydraulic chain saw, holding the timber with his left hand. The saw kicked back and cut his left hand between the index and middle finger. The cut required stitches between the fingers, he was back to work at full capacity 9/17/2015.

HE WAS SHOVELING THE BELT TAILPIECE. HE LOADED THE SHOVEL AND TURNED TO DUMP THE LOAD AND FELT PAIN IN HIS LOWER BACK.

Employee was tapping a bearing onto a drive roller shaft. A small sliver of the bearing chipped off and entered into the left arm, lower bicep area. He went to the ER to have it checked. The doctor removed the small sliver, he is back to work at full capacity.

He was installing metal overcast panels, another worker slid a steel panel onto his left hand. The panel mashed his left thumb, he continued to work the rest of the shift. He went to the doctor after work and they decided it required stitches. He was back to work at full capacity the next day.

Maxium Engineering Contractor's spadman was entering the portal and struck his forehead on the underside of the portal canopy. He stated that he was on his knees when they entered the portal canopy on the two man personnel carrier. The impact caused a laceration above his right eye requiring stitches.

Employee was looking for a busted water line when the fall was found. The fall was located in the return travelway 40 feet inby spad no. 1938. It was reported outside and MSHA was notified. The area was dangered off until inspected. The fall was aprox. 150' long 4-6 foot thick. 55ton jacks and wooden timbers were installed to protect miners and the area affected.

During the preshift exam a roof fall was found in the Primary Escapeway. The fall was reported outside and Msha was notified. The area was dangered off until inspected. He was torquing a fully grouted resin bolt and felt pain in his right ankle. He said it twisted and he sit down. He was brought outside and taken to doctor to have it x-rayed. His right ankle showed to be broken.

EE and co-worker were moving loose bolts off the top of the roofbolter. Co-worker was pulling a bolt out of the bundle, EE was standing behind co-worker and the head of the bolt struck EE above the right eye. He went to the doctor and the cut required a few stitches. He has not lost any work and is at full capacity.

He was pulling miner cable and felt a pop or burning sensation in his left knee.He kept on working and finished the shift. After work he decided to go and have it checked. The doctor told him to work light duty until an orthopedic doctor looked at it. He is currently working light duty outside until his appt. on 11/02/2012.

EE was traveling return and found a roof fall on 3 right panel. It was in No.2 entry around spad number 3642. Fall did not restrict air and was inspected by MSHA rep. No accident or injury was found. Rock was approx, 18' x 50' x 4' thick.

Employee was pulling cable off a roof bolter cable reel, the cable stalled and he fell on his left shoulder. He felt pain but continued to work, he reported it to the superintendent but said he was ok. On 02/27/2012 he went to physician and the doctor scheduled surgery for 03/16/2012. He has not missed any work but will be off for several weeks after surgery to recover.

The section foreman notified the belt man that the belts had stopped. The beltman traveled up #4 belt to #4 drive. He told section foreman the drive would not start, he traveled up #4 belt and notified the foreman the belt looked like it was bound up. The section foreman traveled to #4 belt and found the rock fall one break inby spad no. 1902. The rock was apprx. 8'H X 12'W X 80'L

He was changing a ripper jack on the continuous miner. He was guiding the jack to the alignment holes when the ripper jack slipped and caught his right hand. The chain hoist connected to the jack slipped and allowed the ripper jack to fall.

EE was shoveling loose coal on belt line. He felt a tightness in his lower back. He has been on light duty until 1/4/11. He is currently rec. treatment and will return to work at doctor's release.

Employee was operating the scoop and placed his left hand in a pinch point location. He turned the scoop to the left, pinching his left hand. His middle and ring finger were mashed by the bumper pad on the machine.

EE was pulling a chain while using belt clamps. Ee said he had strained his back. EE said it felt like a pulled muscle. EE called back later that day and said he was going to the doctor. EE left mine site without reporting to the Superintendent. EE came back to work on his next sch. shift. EE did not miss any work until 6/4/2010.

Slipped on mine floor and cut left arm requiring stitches.

A roof fall 2000 ft outby section in return in #1 entry 75 ft outby # 1126 on 1 left submains. Size of the fall: 6 ft high 19 ft wide and 60 ft. long

He was installing roof bolt. He put bolt in hole and rotated with up pressure. When he let head down, bolt and plate came back down also. He had not put glue in hole. He removed bolt, put in glue, reinstalled bolt and rotated. As it was going up, unaware that plate had gaulded (welded) to nut from previous installation, plate rotated with bolt and cut his arm.

He fell down.

Riding 3-wheel mantrip, he hit his head on mine roof.

Driving 3 wheel mantrip outside, steering wheel kicked out of his hands striking him on the hand.

His foot stuck in the mud and twisted his knee.
