INJUREED HAD POSITIONED HIMSELF ON THE OPERATORS SIDE OF THE MACHI NE APPROX 8\ FROM THE MACHINE, WHEN SOME DRAW ROCK & COAL FELL FROM THE RIB STRIKING THE INJURED ON THE BACK & ANKLE.

EMPLOYEE WAS RETURNING FROM LUNCH PERIOD WHEN A BUMBLE BEE HIT HIUM 3 TIMES. INHIS HASTE HE RAN AND DID NOT SEE OBSTRUCTION IN PATHWAY. SH SCRAPED HIS LEFT LEG ON A DISCARDED, DAMAGED MET AL FIRST AID BOX (REQUIRED 9 STITCHES).
