EMPLOYEE TOOK THE ROOF BOLTER TO #7 ENTRY TO LETTHE MINER GO INTO #6 HEADING WHILE TRAMING THE MINER INTO THE #6 THE MINER CAUGHT THE PINNER CABLE AND PULLED IT DOWN. HE WAS WALKING FROM #7 TO #6 TO HANG CABLE BACK UP AND A ROCK FELL FROM RIB TO THE FIRST BOLT HITTING THE EMPLOYEE.

AREA HAD BEEN SUPPORTED WITH 4' FULLY GROUTED RE SIN BOLTS WITH SUPPLEMENTAL SUPPORT CONSISTING OF 9' COMBINATION BOLTS. NO INJURIES, BELT LINE WAS CAUGHT, VENTILATION WAS NOT INTERRUPTED. FALL WILL BE CLEANED UP.

THE ATRS THEN TILTED UPWAR D CATCHING HIS HEAD AGAINST ROOF. BRUISE TO RIGHT SIDE OF FACE [CHEEK BONE] X-RAYS REVIEWED ON 4/22/96 FR NEAR RT EYE. EE WORKED 4-20,21,22 WAS SCHEDULED OFF 3 DAYS. IST DAY MISSED 4/26/96.
