THE INJURED EMPLOYEE WAS LIFTING A REMOTE BOX TO HAND TO THE CONT MINER OPERATOR. AS HE HANDED IT TO HIM, HE FELT A SHARP PAIN IN LOWER BACK.

A ROOF FALL OCCURRED IN BELT ENTRY AT FACE BETWEEN #1 & #2 INTERSECTION. THE FALL WAS APPROX 20FT LONG, 20FT WIDE, & 5FT HIGH. THE AREA WILL BE CLEANED UP & REBOLTED.
