EE WAS INSTALLING A 5' RESIN BOLT, HE PUT 2 HEADER BOARDS ON BOLT TO GET A SURFACE BEARING IN UNEVEN TOP. THE RESIN SET UP TOO QUICKLY CAUSING BOLTER HEAD TO TWIST SUDDENLY AND THE BOLT WRENCH CAUGHT HIS FINGER AGAINST METAL ON BOLT MACHINE
