AS HE FILLED OUT ON THE WORKERS COMPENSATION REPORT: A STEEL SHAFT HAD TO BE LOADED AND I HAD TO PULL, ROLL, PUSH IT INTO THE BACK OF PICKUP WHICH WAS AKWARD AND HEAVY, SO USED MY SHOULDER(RIG HT) AND PULLED MUSCLE OR SUCH FORM BONE. HAD TO HAVE SURGERY. LAST DAY WORKED: 09-09-92.
