EMPLOYEE WAS TRYING TO BREAK PIPE FITTINGS APART USING A PIPE WRENCH AND CHEATER BAR IN A PIPE VISE. THE PIPE BROKE AND THE CHEATER BAR HIT LEFT HAND AND WRIST. CAUSED STRESSED TENDON IN LEFT WRIST.

THERE WAS AN EXPLOSION IN THE VERTICAL SHAFT FROM HYDRO CARBON BLDG THE IGNITION SOURCE IS UNKNOWN. THE EXPLOSION FOR4CED THE MAN CAGE, WHICH WAS PARKED ABOUT 10 FT BELOW THE SHAFT DOORS, TO B REAK THROUGH THE DOORS AND UP THRU THE TOP OF THE HEADRAME. THE MAN CAGE LANDED ON THE ROOF OF THE HOIST HOUSE. WITHOUT THE HOIST CAN NOT GET UNDERGROUND TO INVESTIGATE.

WHILE WELDING PIPE AT SHAFT COLLAR SOME SPARKS ACCIDENTLY IGNITED RAG USED AS FIL MATERIAL IN END OF PIPE. RAG THEN FELL DOWN TO BOTTOM & IGNITED CRUDE OIL. DAMAGE FROM WAER PUT INTO SHAFT TO PUT FIRE OUT TO EQUIPMENT & SOME LOSS OF PRODUCTION.
