WORKER WAS OPERATING A D6M CAT CRAWLER DOZER AND BACKED OVER HIGHWALL IN MINE INTO WATER-FILLED PIT AND DIED DUE TO DROWNING.

MOVIGN MATERIAL TO CRUSHER SITE. HAD JUST BEEN LOADED ON A 30 TRUCK. DROVE OFF ROADWAY, TRUCK SAID OVER ON SIDE IN WATER. HOSPITAL WILL KNOW CAUSE OF DEATH.
