EE WAS ATTEMPTING TO SET AN ACETELYNE CYLINDER TO THE GROUND. THE CYLINDER SLIPPED OUT OF HIS HANDS AND FELL ON HIS FOOT, STRIKING HIM ON TOP OF HIS FOOT BEHIND HIS STEEL CAP ON HIS SAFETY SHO ES. THIS CAUSED A FRACTURE OF THE METATARSAL.

EE WAS ATTEMPTING TO LOAD A LARGE ROCK INTO TRUCK WHEN THE ROCK SHIFTED AND CAUSED THE REAR OF LOADER TO RISE UP. THE ROCK SHIFTED AGAIN CAUSING THE LOADER TO DROP SUDDENLY. THIS CAUSED HIM TO BUMP HEAD ON CAB ROOF AS HE WAS THROWN UP OUT OF HIS SEAT. THIS RESULTED IN A MILD CONCUSSION.
