Employee found the loader operator unresponsive and called the plant foreman. The plant foreman entered the loader cab and placed the equipment in park and shut the motor off. EE was unable to get any vital signs from the operator and immediately dialed 911. The local fire department responded, and the loader operator was pronounced dead at approximately 12:35 pm.

Employee was operating a loader when they ran over a chunk of asphalt in loader causing their body to shift abruptly in the cab and causing a right shoulder strain.

Climbing down ladder of a 982M loader and felt pain in left foot which led to a sprain/strain of left foot.

IW was using a shovel to remove material from a lime stacker belt. A few hours after the task was completed, IW stated that their shoulder was hurting. IW was seen at the clinic and diagnosed with a sprained left shoulder joint. IW returned to work the following day on light duty.

The employee parked loader at the ready line parking lot & was exiting the loader to go clock out as the employee was at the end of shift. While exiting the loader, stepped on the ground felt a pain in right foot. There were no adverse conditions or hazards associated with the area employee exited at. The ground was dry & even and employee used three points of contact.

Employee was exiting the water truck EE was driving when EE fell and hit the ground resulting in a left upper leg fracture. Employee was taken to hospital where EE had surgery to repair EE's fractured leg. Employee was then admitted to an in-patient physical therapy unit.

The Employee was preparing to start up the lime AB system when some quicklime became dislodged and the wind blew the quicklime into the face of the EE. The EE used the eyewash system for a few minutes to wash out eyes and face and had not further issues until the EE tried to remove contact lenses which caused a corneal abrasion due to the quicklime

Employee was moving a steel box with another box on top. As the employee was moving the bottom box, the top box shifted and fell over onto the employees fingertips, causing a laceration to the left index finger requiring stitches.

Employee was lifting a return roller into position above his head when he felt a pop in his upper back between his shoulder blades

Employee was using a cutting wheel on a grinder to cut 1x1 inch rectangle steel. The EE stated that the wind kicked up blowing dust into his eyes and he lost sight of the cut off wheel causing it to hit and kick off the steel and hitting the employees right leg just above the knee causing a 3 inch laceration. This laceration required 15 stitches to close.

EE was using a razor knife to cut flashing, when the knife slipped and hit the EE in the Right thigh/hip area. The injury required stitches to close.

EE was cutting beltline with razor utility knife. EE was pulling the knife towards himself when the knife slipped off the rubber and hit the EE's right thigh about six inches above the knee causing a laceration.

The EE was using a skid steer to clean when it became stuck. The EE exited the skid steer and then re-entered the skid steer from the side rather than the front. The EE stepped onto the front right tire and climbed into the skid steer, and caught his left arm on a broken headlamp mounted on the front of the ROPs, lacerating his arm which required sutures.

EE WAS RUNNING APPROX 200 FT TO SHUT OFF A PUMP WHEN HE FELT A PAIN IN HIS LEFT CALF MUSCLE. DIAGNOSIS: TORN L CALF MUSCLE.

Employee was climbing a dirt embankment instead of using the ramp. He twisted his left foot and complained of a sharp pain in his heel. Employee asked to see a doctor 8/22/07. Note: due to conflicting stories of what body part is injured, this injury is being questioned. This injury occurred one week after a previous injury and one week after his hire date.

This was the employee's first day of work. He spent most of the day in the safety training room and a few hours in the field training with a trainer where he was shoveling. Employee claims he shoveled excessively and strained his wrist. Employee asked to see a doctor 8/22/07. Note: due to conflicting stories and reports, this injury is being questioned.

EE WALKED OVER THE TOP OF A BELLY DUMP UNLOADER AND STEPPED IN A RECESSED AREA TWISTING HIS RIGHT ANKLE.

EMPLOYEE WAS LIFTING ARM ON SPLITTER BOX TO CHANGE MATERIAL FLOW. WHILE LIFTING ARM, HE FELT A PAIN IN HIS LOWER BACK, RIGHT SIDE.

EE REPORTED ON 8-24-99 THAT SHE HAD LOWER BACK PAIN FROM OPERATING LOADER. SHE REPORTED SHE COULD NOT MOVE SEAT UP AND FORWARD TO GET COMFORTABLE. INVESTIGATION FOUND THAT SEAT WAS 2 MONTHS OL D. ALL ADJ. WORKED PROPERLY. NO INDICATION OF CAUSE OF INJURY. SHE REQUESTED THAT DR PUT HER OFF WORK ON FRIDAY AUGUST 27,1999. UNABLE TO ESTABLISH ACCIDENT LEADING TO INJURY.

EE WAS DRAGGING CRANE CABLE BEHIND HIM TO MOVE A CONVEYOR. HE FELT A SHARP PAIN IN HIS KNEE.

HE WAS SHOVELING WHEN SHOVEL HIT A LARGE HIDDEN ROCK. IT JARRED HIS LEFT ARM. WHEN SYMPTOMS DID NOT RESOLVE, HE FILED AN INDUSTRIAL ACCIDENT CLAIM ON 8-24-98.

EMPLOYEE WAS CLEANING GRIZZLEY. HE HIT HIS RIGHT SHIN WITH 8 LB. HAMMER. HE REFUSED MEDICAL ATTENTION AT THAT TIME. HE REFUSED MEDICAL ATTENTION ON TWO MORE OCCASIONS. HE REQUESTED MEDICAL ATT ENTION ON 3-1-96.

EMPLOYEE WAS DESCENDING STEPS AT CRUSHER PRIMARYSCREEN. SPRAY BARS USED FOR DUST CONTROL HAD WET STEPS AND GROUND. AS EMPLOYEE REACHED LAST STEP, HE SLIPPED ON MUD. HE DID NOT FALL, BUT STRAIN ED LOWER BACK.

INJURED WAS TRYING TO PUSH FLOP GATE OVER WHEN HE FELT PAIN IN BACK, HE WAS USING HIS FEET AND SUPPORTING HIMSELF WITH HIS BACK. BACK STRAIN- LIGHT DUTY APPROX 10 DAYS.

EE BELIEVES HE HIT HIS ELBOW ON HAND RAIL WHILE SHOVELING MATERIAL OFF OF CATWALK CAUSING HEMOTOMA OF RIGHT ELBOW.

INJURED WAS SHOVELING MATERIAL FROM AROUND TAIL PULLEY WHEN HE FELT A PAIN IN LOWER BACK RESULT ING IN LUMBAR STRAIN.

HE WAS TRAVELING APPROX. 25 MPH IN LOADED BELLY DUMP. HE HIT A DIP IN THE HAUL ROAD CAUSING HIM TO HIT HIS HEAD ON STEERING WHEEL. EXCESSIVE SPEED AND FAILURE TO CONTROL VEHICLE.

EMPLOYEE WAS REPLACING SCREEN CLOTH @ WASH PLANT. AS HE WAS PULLING SCREEN INTO POSITION HE CUT HIS INDEX FINGER ON HIS LEFT HAND. REQUIRED STITCHES.

EE SLIPPED AND FELL BACK HITTING HIS TAIL BONE ON A PIECE OF CONCRETE. RESULTING IN A CONTUSION TO TAIL BONE.

EE WAS CLEANING N.A. TUNNEL. AS HE LIFTED WHEELBARROW TO MOVE, HE FELT PAIN IN LOWER BACK.

EE WAS HAULING MATERIAL IN HAUL TRUCK FROM PIT. AS HE APPROACHED A CURVE IN ROAD, HE BOUNCED IN CAB AND FELT A TWIST IN LOWER BACK.

I NAME WAS STANDING ON BACK OF FLATBED MECHANIC TRUCK REPAIRING A PULLEY ON THE ABC STACKER WHEN HE TURNED TO FACE REAR OF TRUCK HE STEPPED INTO A H OLE IN THE BED OF THE TRUCK CAUSING HIM TO LOSE HIS BALANCE & FALL OFF OF THE TRUCK

REMOVING A DRIVING GEAR FROM VIBRATING UNIT ON FEEDER.COUNTERWEIGHT SHIFTED AND AS GEARS MOVED,PINCHED HAND THROUGH WORK GLOVES AND CAUSED FRACTURE TO LITTLE FINGER RIGHT HAND BETWEEN KNUCKLE AND FIRST JOINT.SIMPLE FRACTURE.

HE WAS REMOVING A ROCK FROM BEHIND THE SIDE WEAR PLATE ON THE PAN FEEDER. HE WAS USING A PRY BAR TO PRY PLATE AWAY FROM ROKS. AFTER ROCKS WERE REMOVED, THE PLATE SPARNG BACTHING HIM BETWEEN THE BAR AND SIDE OF FEEDER WALL.

CLAIMANT WAS HE5PING REMOVE A ROCK THAT WAS WEDGED IN BOX ON A FIELD CONVEYOR IN THE PIT WHEN ROCK CAME LOOSE CLAIMANT LOST HIS BALANCE & FELL OFF BOX TO GROUND
