CalPortland Company · Caught in, under or between a moving and a stationary object

The Aggregate Foreman was attempting to attach a trailer carrying a compressor to EE's F-150 work truck. While trying to set the trailer hitch onto the hitch ball of the truck, the trailer hitch came off the ball and moved forward. The employee got EE's finger caught between the hitch and the bumper of the truck, causing a laceration to EE's left index finger.

Employee found the loader operator unresponsive and called the plant foreman. The plant foreman entered the loader cab and placed the equipment in park and shut the motor off. EE was unable to get any vital signs from the operator and immediately dialed 911. The local fire department responded, and the loader operator was pronounced dead at approximately 12:35 pm.

IW was using a shovel to remove material from a lime stacker belt. A few hours after the task was completed, IW stated that their shoulder was hurting. IW was seen at the clinic and diagnosed with a sprained left shoulder joint. IW returned to work the following day on light duty.

The employee parked loader at the ready line parking lot & was exiting the loader to go clock out as the employee was at the end of shift. While exiting the loader, stepped on the ground felt a pain in right foot. There were no adverse conditions or hazards associated with the area employee exited at. The ground was dry & even and employee used three points of contact.

Employee was exiting the water truck EE was driving when EE fell and hit the ground resulting in a left upper leg fracture. Employee was taken to hospital where EE had surgery to repair EE's fractured leg. Employee was then admitted to an in-patient physical therapy unit.

The Employee was preparing to start up the lime AB system when some quicklime became dislodged and the wind blew the quicklime into the face of the EE. The EE used the eyewash system for a few minutes to wash out eyes and face and had not further issues until the EE tried to remove contact lenses which caused a corneal abrasion due to the quicklime

Employee was using a cutting wheel on a grinder to cut 1x1 inch rectangle steel. The EE stated that the wind kicked up blowing dust into his eyes and he lost sight of the cut off wheel causing it to hit and kick off the steel and hitting the employees right leg just above the knee causing a 3 inch laceration. This laceration required 15 stitches to close.

The EE was using a skid steer to clean when it became stuck. The EE exited the skid steer and then re-entered the skid steer from the side rather than the front. The EE stepped onto the front right tire and climbed into the skid steer, and caught his left arm on a broken headlamp mounted on the front of the ROPs, lacerating his arm which required sutures.
