EMPLOYEE WAS TRYING TO REMOVE A LARGE WEDGED ROCK IN THE JAW CRUSHER. AS HE WAS PUSHING THE ROCK DOWN INTO THE CRUSHER WITH HIS FOOT, HE LOST HIS BALANCE AND SLIPPED INTO THE CRUSHER THUS A MPUTATING THE SECOND AND THIRD TOES, THE TOP OF THE FOURTH TOE AND BREAKING IT ALSO ON THE LEFT FOOT.
