Employee was holding a lug in the left hand and cutting it with a porta band saw in the right hand. The porta band saw slipped and lacerated the left index finger and slightly cut the long finger. Employee received six sutures and was returned to work full duty, no restrictions, no prescriptions.

While drilling holes for roof bolts and mesh with a jackleg drill, a section of roof fell on two miners. The individual for this report suffered fractures to the tibia, fibula, and 3 vertebrae.
Employee was installing shaft and bearing housing. Mounting bolt was not aligned. Employee used a pry bar to lift the housing in order to reposition the bolt. The employee reached under the housing and the pry bar slipped. Fourth finger was pinched between housing and mounting surface fracturing tip of finger and requiring stitches.
Employees working at the end of the day. Employees wearing full-face PAPRs. Employees noticed fan blades making contact with housing. Employees were on opposite sides of the fan unit. One employee rotated the fan side to find contact areas. Employee on the other side attempted to assist rotation and pinched finger between the pulley and fan belt.

Employee stepped over process line during clean up activities and rolled ankle. Felt they could self treat until 7/20/18 when they requested to see Dr. Evaluation with the Dr. on 7/20/18 discovered non-displaced fracture to distal fibula and placed on restrictions.
Employee was removing liner from ball mill. A pneumatic jack was holding the mill in place. The jack failed and the mill turned 180 degrees. An employee inside the mill was injured from the fall/flying debris.
Employee was pushing a pallet into the bulk sacker and pinched a thumb on the rollers. 8 stitches was placed on the thumb.

while working on replacing an arm of the auto package system employee had to kneel on a chain conveyor. when employee moved off the conveyor for break, he felt stiffness in his back. employee did not receive medical attention until 7/10/14 as he thought pain would go away.

Employee was attempting to close a railcar door latch. The latch wouldn't go down to the locking position. Employee then, using heel of his foot, kicked the latch to close completely.On 5/22 employee received x-ray for foot being swolle,diagnosed with inflammation in foot.
Employee went to turn around after picking up tools and felt sharp pain in knee. employee received MRI on May 16th indicating a tear in his meniscus.
Employee was attempting to cut a rubber hose with a utility knife. Employee was cutting towards his body and broke through the hose and his motion caused him to cut his leg with the utility knife. Cut was small and clean, back to work in 2 hours from hospital

Employee was moving 50 lb bags on a pallet when he jammed his wrist. Did not seek medical treatment until 3/14/11 and became reportable when employee went to physical therapy on 3/21/11. Employee stayed at full duty the whole time.
Customer's over the road truck driver was found unconscious near the truck scale.
Maintenance employee was preparing to weld on 6DC9 dust collector. He was carrying a six foot step ladder on his right shoulder and a five-gallon bucket in his left hand. While walking around the dust collector, the employee fell on a maintenance access ladder. Employee suffered a contusion to the right elbow.

Employee reported pain in right shoulder on 9/10/09 but didn't want to seek medical treatment as he though it was something minor. On 9/29/09 employee was attempting to release hand brake on a railcar when he felt a sharp pain in his right shoulder. Employee sent to medical center on 9/30/09 and was diagnosed with sprain in right rotator cuff.

Employee was cutting skaker deck apart to send off for repair. While attempting to remove motor from frame, part of the beam holding the motor would not come loose, as the cuts were not completely through the frame. Employee bent down to grab cutting torch when the weld broke causing the edge of the motor to contact the left side of employee's face.
Employee was wiping up oil from maintenance shop floor, while another employee was stacking three wooden pallets up. While cleaning floor, one pallet fell onto left index finger cutting it.

Feedbox on #5 mill was jammed up with ground silica material. Employee shut down mill and cleaned feedbox out. Employee started mill back up after completing cleaning. Feedbox again jammed up, due to fines from seperator. Air blast occurred splashing product into employee's eyes.
Employee was repositioning a 50lb bag of sand when he stepped on a pallet lift. The lift had a bolt break. The lift fell on the employee's foot. He went to doctor on 5-7-07. Doctor said looks like swelling; come back if anymore problems. Emp. went back during Sept. Doctor said surgery would take care of cyst on 9-10-07.

Employee was going to empty a wheelbarrow of sand, while in the process of pushing the wheelbarrow, the employee stepped over a piece of grating that was bowed. The employee twisted his knee when he stepped on the grating. Employee did not go to doctor until 12/11/06. Employee waited to see if the swelling went down before going to see a doctor.
emp. was attempting to release hand brake on railcar. He grabbed a 3-foot bar and placed to release brake. emp, felt pain in left shoulder. Emp, went to doctor six days later. doctor said no findings, in 04/07 he asked for 2nd opinion. worker's comp and attorney have been disbuting claim. finally a specialist elected to have surgery. 1st day of work missed was 12/3/07
Employee was moving pipe with back hoe equipment, carrying pipe between the knuckle and using the bucket for support. Employee's safety glasses became fogged up due to cold weather. Employee stopped to wipe off glasses when the pipe fell into the water/sand area, which splashed up into employee's eye.

EE WAS SLOADING A TRUCK FROM A #1 SILO. HE WAS PULLING ON THE HOLE/CHUTE COVER UP WHEN HE FELT A PULL IN THE LEFT SIDE OF HIS NECK
EE WAS IN THE PROCESS OF PLACING A 50 LB BAG OF CALCIUM CLORIDE ON THE GROUND WHEN HIS FOOT SLIPPED ON THE ICE AND FELT A PAIN IN THE GROIN AREA.

FOR EVA LUATION. EMPLOYEE WAS DIAGNOSED WITH BURSITIS BOTH SHOULDERS BUT NOT TREATED FOR CONDITION. UPON RETURN VISIT TO CO. DR. DEC 08, PHYSICAL THERAPY WAS PRESCRIBED.
EMPLOYEE WAS WALKING DOWN A WET, CLAY-COVERED BANK TOWARD THE FLOATING BARGE IN THE PLANT QUARRY. HE WAS CARRYING A BUCKET OF TOOLS WITH WHICH TO WORK ON A MOTOR. HE SLIPPED ON THE WET CLAY, F ALLING TO THE GROUND, WITH HIS LEG "SPLIT".
EMPLOYEE WAS WORKING IN THE ELEVATOR PIT, CLIMBED OUT TO GET A SHOVEL AND STRUCK HIS HARD HAT ON A STEEL BEAM ABOVE HIS HEAD.
EE WAS TIGHTENING THE BRAKE ON A RAILCAR BY TURNING THE BRAKE WHEEL HIS RIGHT HAND SLIPPED OFF THE BRAKE WHEEL STRIKING AGAINST A STEEL SUPPORT DIRECTLY BEHIND THE BRAKE WHEEL CAUSING A STRAIN TO HIS RIGHT WRIST.

HE WAS PULLING ON ROPE TO OPEN THE LOADING CHUTE FROM #2 SILO. THE CLEVIS PIN CAME LOOSE, CAUSING THE ROPE AND PULLEY TO SUDDENLY COME FREE. HE FELL BACKWARDS OFF THE RAIL CAR.
