An employee was helping change a screen on the screen deck. The employee had another employee holding the screen to help out. While trying to climb out of the screen deck, the screen was dropped and hit the employee in the head, resulting in 5 stitches being needed.

Employee was pre shifting vehicle after safety meeting, and cleaning windows on Volvo A30 haul truck, started to wash the windows, standing next to the fueling spout on the flat are provided for standing, slipped on fuel residue and fell approx 6 feet to the ground on his lower back and butt. Employee also had a headlamp on, the sun was just starting to come up.

While loading boulders in a 30 ton haul truck a rock slipped out of the thumb of the excavator allowing it to fall into the bed of the truck (approx. 5-7 feet) jarring the truck operators neck.
