Cutting rubber skirting with utility knife, cut into self instead of away knife bounced and into upper part right leg, 3 inches along about 1 inch deep requiring 4 stitches. Employee returned to work full duty.

Employee was pre shifting vehicle after safety meeting, and cleaning windows on Volvo A30 haul truck, started to wash the windows, standing next to the fueling spout on the flat are provided for standing, slipped on fuel residue and fell approx 6 feet to the ground on his lower back and butt. Employee also had a headlamp on, the sun was just starting to come up.
