Employee was standing on back of welding trailer loosening mounting bolt off of a 1400 screen plant when the 1/2in drive 3/4in socket slipped off of the loose nut and came in to contact with the top front left tooth causing it to chip a 3rd of the tooth off.

Employee reported injury on 2/5/16. Employee states on 12/8/15 she felt a pop on her right knee when closing the door to the scale house.
Employee was helping change the shoes on the vsi crusher. Weather was clear and sunny, about 75°F.
Employee needed to load the air compressor in the back of his truck to take to another location. He had oxygen and acetylene bottle in the back of his truck and needed to move them out of the way. He picked up the acetylene bottle to lay it back down in the bed, when he did this he dropped the bottle of top of his foot.
Clear skies and 60°F with W winds blowing approximately 10-15 mph. Employee was waiting for a 1-1/8" socket to tighten bolts on a belt clamp. When he looked up to grab the socket and the wind blew some dust into his eye.

Miner was completing screen maintenance on a portable screen. Miner was tightening the fasteners of the screen retaining brackets and the wrench slipped off of the fastener. Miner struck an adjacent fastener with his right ring finger when the wrench slipped. Miner suffered a small contusion under his right ring fingernail.
Was unloading rock screen from a bucket of a loader. As they was lifting the screen the other man lost his grip at this time the screen caught the mans glove and pulled him to the ground causing him to hurt this hand.
