EE WAS INT HE PROCESS OF PUTTING BAGS-FULL OF COLLECTED DUST INTO THE LOADER. BUCKET FOR DISPOSAL. WHILE DOING THIS THE LOADER BUCKET SHIFTED PINNING HIS RIGHT FOOT BETWEEN THE BUCKET AND THE GROUND. THIS ACTION MASHED HIS STEEL TOED BOOT ONTO HIS FOOT CAUSING THE FRACTURE OF HIS BIG TOE AND THE TWO NEXT TO IT.

USING PIPE WRENCH TO TIGHTEN 2" JOINT OF PIPE.APPLIED PRESSURE TO WRENCH WITH FOOT,IT SLIPPED CAUSING HIM TO STRIKE KNEE AGAINST GROUND.DIDNT NOTICE PROBLEM SO DIDNT REPORT INCIDENT UNTIL 12-1 0-90 TO SUPV AND MED DEPT.REFERRED AT THAT TIME.

EE WAS MOVING A RAILROAD TIE AND A SPLINTER FROM THE TIE EMBEDDED IN HIS RIGHT FINGER. HE REMOVED THE SPLINTER AT THAT TIME, AND DID NOT REPORT IT AS HE FELT IT WAS MINOR. THE NEXT DAY IT BECA ME SWOLLEN AND HE REPORTED IT TO THE CO NURSE, AND WAS REFERRED FOR FURTHER MEDICAL ATTENTION ON 8-2-90.

TRANSPORTED TO DR & RELEASED BY DR TO COME BACK TO WORK NEXT M ORNING LIGHT DUTY. RELEASED FOR FULL DUTY 8/9/88. EMPL WAS TAKING PRESCRIPTION MED WHICH MAY HAVE CONTRIBUTED TO HEAT EXHAUSTION.
