EE was transferring drill bits from the truck bed to the drill deck. EE isn't sure how it happened, but the drill bit ended up smashing EE's left thumb between the drill bit and the drill deck.

Employee was dislodging cheek plates from a crusher. A porta power was requested to assist. The employee placed the porta power on a ledge for preparation. The porta power fell off the ledge approximately 16" striking the employee in the right arm.

Employee was carrying a 3-ton jack from the maintenance truck to a conveyor. Employee states that he stepped in a hole and twisted his right knee. On 8/28/2015 he was seen by an orthopedic doctor who recommended surgery. Surgery occurred on 9/24/2015.

An employee and a contractor were welding inside a crusher. After the shift, the employee noticed that his eyes were stinging. His eyes were flash burned by the exposure to the other welder' s arc.

Employee was helping unroll spool of wire rope for the dragline. As they neared the end of the rope, it slipped out of the attaching staple, and struck the employee in the mouth.
After a repair on the dragline, the employee complained of pain in his shoulder. He was examined by a doctor, and diagnosed with a strain. He was allowed to return to normal work duties. On May 23, 2007, he complained of more pain, and returned to the doctor. At that time he was excused from work pending further medical tests.

WHILE GAINING ACCESS TO MACHINE, EE WAS ON STAIRS PULLING HIMSELF UP BY HANDRAILS. HIS FEET CAME OUT FROM UNDER HIM DUE TO OIL COVERED WITH DIRT ON TRACKPADS. BODY WEIGHT WAS HELD UP BY ARMS. HE FELT A PAIN FROM HIS RIGHT SHOULDER. THIS CASE WAS TREATED AS A FIRST AID CASE UNTIL 1/13/05. AT THAT TIME AN MRI SHOWED A TORN ROTATOR CUP. PHYSICIANS ARE RECOMMENDING SURGERY.
