HE HAD FAILED TO RELEASE THE END-GATE LATCH WITH THE BOX IN THE DOWN POSITION & HAD RAISED THE BOX WITH A LOAD ON IT & THEN CRAWLED INTO THE BOX TO ATTEMPT TO RELEASE THE LATCH, HE FELL TO THE GROUND IN THE PROCESS OF TRYING TO RELEASE IT. WHEN HE FELL HE FELL ON HIS BOTTOM & TRIED TO CATCH HIS FALL WITH HIS ARM. IN THE PROCESS HE BROKE HIS LEFT RADIUS RIGHT TIBIA, FRACTURED HIS ST
