Wednesday, 11/09/2011, the individual was inside the screen pushing up on a panel and felt pain in his shoulder. He felt the injury was insignificant and didn't report it to management until 11/14/2011. He was referred to his primary care physician immediately who felt that it was minor.

HE (MECHANIC) WAS WORKING IN REPAIR SHOP (OFF-SITE OF MINE). HE WAS PLUGGING A TIRE. THE TIRE TOOL BROKE AND WENT INTO HIS RIGHT INDEX FINGER.
