EMPLOYEE WENT TO DR. WITH A CASE OF SEVER HEMORRIODS. EMPLOYEE HAD RETURNED TO FULL TIME ELMPLOYEMENT THE 1ST WEEK OF APRIL. EMPLOYEE WAS TOLD BY DR TO SEE IF WORKMAN'S COMP WOULD COVER HIS HEMRROIDS. WHICH BECOME WORSE WHEN DRIVING TRUCK.

EMPLOYEE PICKED UP A HEAVY PULLEY WHILE MAKING. REPAIRS ON GRAVEL PROCESSING PLANT, EMPLOYEE THINKS HE STRAINED IN LIFTING THE PULLEY, HE THINKS IT CONTRIBUTED TO GIVING HIM A TRANSLUMINAL CYS TIC MASS IN THE SPERMATIC CORD.
