Attempting to operate loaded truck on an incline when it overturned. This occurred at night. EE had seatbelt fastened and received bruising from belt. Due to size of drive the past was removed from the truck so ee could exit from vehicle. When upright truck started and no damage was done & fan on motor. Drive line was damaged and windshield broke, driver side door also damaged.

Employee was unplugging chute at quad silo using a slate bar. While prying on the material, it came loose mashing his right hand between the bar and the edge of the chute. This caused a laceration on top of his right index finger between the knuckle and first joint. This required 5 stitches.
